chore: KCP 0.26 — repair manifest YAML, add serving endpoint binding

- strip malformed empty content_hash blocks (broke YAML parsing)
- kcp_version -> 0.26
- serving block (SPEC §3.12): raw manifest URL + mcp.cantara.no/mcp
