Feat: create a bug issue report template

Description:

We need to add a dedicated bug report issue template to standardize how users and contributors report bugs. Makes it easier for users to submit an issue related to bug finds.

Details:

Include the following fields:

• Title
• Description*
• Steps to reproduce
• Expected vs actual behavior
• Environment details
• Screenshots or logs
• Additional Note

The template automatically labels new issues as bug

Why:

Having a consistent bug report format will make it easier to reproduce and fix issues efficiently.
