Consolidate the 4.0 migration guide from files in docs/4.0-migration-guide in TF-PSA-Crypto and mbedtls.

The result may be a single document or separated in a few topics (e.g. build, configuration, crypto, X.509, TLS), whatever makes the most sense. It's unclear at this point whether we'll want a single migration document, or one for Mbed TLS and one for TF-PSA-Crypto.

This task should be done near the end of the project, when the changes have stabilized.
