current location is not being picked up when issues are reported at https://github.com/apache/pouchdb/issues/new
added basic "frontmatter" to achieve "✔️ checkmark in the community profile checklist" per https://docs.github.com/en/communities/using-templates-to-encourage-useful-issues-and-pull-requests/manually-creating-a-single-issue-template-for-your-repository
