Make the list values tie to some list of address info on agencies

Should have

Human Readable Version in Dropdown
(optional) id/shortname for mapping to addresses
One liner address for agency

ex.

Tulsa Police Department
TPD
Tulsa, Oklahoma 74103

populates in dropdown as
<option value=TPD>Tulsa Police Department</option>

Populates in Document as

Tulsa Police Department
Tulsa, Oklahoma 74103
