feat(portal): create event registration form

Google Doc's can't be branded to be cuHacking themed.

Describe the solution you'd like
We should have some sort of event registration form form our own portal. This event registration should take info from the user profile so that they don't have to retype.

Describe alternatives you've considered

• Google forms, not branded and harder to manage registrants
