Document instructions how to rename an organization #283

This appears to be a three-step process across two different repos:

  1. Do the rename within hub.json
  2. Add a redirect within data/packages/{ old_org }/{ package }/index.json
  3. Add the old org name to data/blocklist.json

Open two pull requests:

  1. https://github.com/dbt-labs/hubcap for step 1
  2. https://github.com/dbt-labs/hub.getdbt.com for step 2 and 3
