msigs not showing updated msig count

Some WP have the needed signatures, but still show as pending. They are even approved and executed, but still shown as if some signatures are missing.

I have seen this as well. It show's 8/9 on the executed page, but blocks info shows 9 signatures.

Another similar note - There are currently 2 msigs populated as OPEN in my UI with 9/9 signatures. But when I go to execute I get a message that I believe indicates the msig as already been executed.
