Error editing file with ChatGPT

Hello, I'm evaluating cmd and despite being a bit slow, I'm encouraged by the functionality and UX. However, I'm suddenly receiving the following error after previously verifying organization and using cmd for over an hour and multiple chat messages:

"Error editing file"
"The content of /Users/abc/Documents/ xyz/Projects/Abc/Abc.xcodeproj/project.pbxproj has changed since it was last read. Re-read the relevant sections first."
"Error sending message: Your organization must be verified to stream this model. Please go to: https://platform.openai.com/settings/organization/general and click on Verify Organization. If you just verified, it can take up to 15 minutes for access to propagate."

The organization was previously verified. Not sure of the cause or fix.

I'm using Xcode RC Version 26.0 (17A321).

Restarting Xcode and cmd resolved the issue.

I was not aware of this specific error with Open AI. I'll look into it.

"Error editing file"

Currently, I raise an error if the agent edits a file it has not read yet. This is to help ensure that edits are relevant. The agent should automatically retry (ie usually it decides to read the file and continues the edit).

Did this error happen after the agent read the file (so this would be a bug); or was the error surprising but not a bug given the above description?
