Section 'Setting up and configuring PuTTY on a Windows system' should also include a link to document 876480 "SSH Tunnel configuration for use with IBM i Access Client Solutions" at https://www.ibm.com/support/pages/ssh-tunnel-configuration-use-ibm-i-access-client-solutions
Additionally, Step 5 should include that the Destination port be localhost:port_number and include the screenshot that I've attached which is from that other document which clarifies which boxes to check and how the other forwarded ports should look like.
