This is a problem I have encountered for both Cline/RooCode and OpenHands from time to time. Sometimes I think that not all of history can be packed into the context window, causing this problem... or maybe the temperature was wrong. Repeating the same mistake without some help with some sort of "what did not work" scratchpad.
