fix: improve light mode readability and console UI fixes

- Installer: replace dim white with bright_black for light terminal readability
- Console Modal: use createPortal to fix activation dialog clipped by sidebar
- Console UI: bump low-contrast text-base-content/20-30 to /40 across views
- Console UI: replace hardcoded dark-only hex colors with DaisyUI semantic classes
- Console UI: use 500-weight Tailwind colors for dual-theme compatibility
- Worker: add fail-open error handling for hook subprocess errors
- Spec commands: add approach selection step with trade-offs to plan templates
- Settings: enable auto-memory (CLAUDE_CODE_DISABLE_AUTO_MEMORY=false)
