Command handlers get an ActionContext (instead of just the CommandHandlerContext)
Remove special executDispatcherCommand and use the standard helper from the SDK.
Switch most dispatcher command handlers to use ActionIO. Remove RequestIO API that is no longer used.
Add block append mode for appendDisplay
