Issue summary

Codex CLI 0.141.0 crashes unexpectedly on an Intel-based Mac while it is running.

After the crash, zsh prints the following message:

[1]    4460 trace trap  codex

The process is terminated by SIGTRAP instead of exiting normally.

This happens during an active Codex session. There is no useful error message displayed in the Codex TUI before the process exits.

Steps to reproduce

1. Install Codex CLI 0.141.0 using the standalone installer.
2. Confirm the installed version:

codex --version

Output:

codex-cli 0.141.0

3. Start an interactive session:

codex

4. Submit a task that causes Codex to inspect files or execute shell commands.
5. Continue using the session for a short period.
6. Codex suddenly exits and zsh displays:

[1]    4460 trace trap  codex

Actual behavior

The Codex CLI process crashes with SIGTRAP / trace trap during execution.

The terminal returns directly to the shell, and the active Codex session is interrupted.

Expected behavior

Codex CLI should continue running normally and should not terminate with SIGTRAP on Intel macOS.

If an internal operation fails, Codex should display an error and keep the session running instead of crashing the entire process.

Regression information

The problem occurs with:

codex-cli 0.141.0

As a temporary workaround, I downgraded to:

codex-cli 0.140.0

Please investigate whether version 0.141.0 introduced an Intel macOS compatibility, runtime, JIT, code-signing, or executable-memory regression.

macOS may have generated a diagnostic report under:

The relevant exception appears to be a SIGTRAP / EXC_BREAKPOINT crash.

Additional information

The CLI was installed alongside an npm installation, but the executable that crashed was confirmed to be:

/Users/houshuai/.local/bin/codex

The npm-installed version was located at:

/Users/houshuai/.nvm/versions/node/v22.17.1/bin/codex

The issue is specifically observed when running the standalone Codex CLI 0.141.0 executable on an Intel-based Mac.
