When using the source type "SQL query" under "Settings", it is not possible to enter a correct query for the list the toolbar is based on. Even the prefilled example query is not working!
The problem seems to be a wrong value for minimum and maximum columns. Both values are currently set to 2. This is obviously wrong, since the list definition takes 2 column values defining menu option icon, link, and description.
