ENH: Big MVC refactor.

Collect the component hierarchy into a single DebugModel class. All of the
views sit on top of DebugModel. Among other things, this allows the
communication necessary to react to changes with Include and Inline.

The top-level window geometry is now persisted in ~/.enthought/enaml_debug/
