Table of Contents Items should link to song pages in PDF #47

Given I am looking at the Table of Contents in one of the PDFs
When I click/tap on a song title
Then I should be taken to the page for that song

Bonus: Provide a link back to the ToC on each page. This will allow people to get back to the ToC to find the next song.
