Found this repo via the persistent-context-across-sessions angle — really cool problem space. Sharing a complementary open-source hook that addresses a related pain: when Claude says "done" but did not actually verify the work.
https://github.com/ianymu/claude-verify-before-stop — a Stop hook that blocks session-end if files changed but no VERIFIED log entry from the last 5 min exists. Forces explicit verification logging (which doubles as audit trail).
The pattern dovetails nicely with claude-mem: claude-mem preserves context across sessions, verify-before-stop ensures the context being preserved actually reflects verified work. State files live in
.claude/state/for both.MIT, ~50 lines bash. Also published a Claude Code Hooks cheat sheet at https://gist.github.com/ianymu/1eb7238b2bac24dc9fdb3784a13c0a26 covering Stop/PreToolUse/PostToolUse contract + the 8-block cap gotcha.
Would love feedback if you have thought about the verification-logging dimension in claude-mem scope.
