This is more-or-less a resubmission of #290.

adds definitions for cl_khr_unified_svm (see add cl_khr_unified_svm to the main OpenCL specification and XML file OpenCL-Docs#1452)
adds CL_COMMAND_BUFFER_STATE_FINALIZED_KHR (see setting kernel arguments before recording it in command buffer OpenCL-Docs#1382)
does not include double typedef for cl_icd_dispatch (see Remove require of cl_icd_dispatch to avoid double typedef OpenCL-Docs#1458)
