Fix/reauth on auth failure - #323

If the provider revoked the token while Home Assistant was running, the library only logged an error and the entities went quiet. Register the library's auth failure callback once setup has succeeded so Home Assistant asks for new credentials. A failed login during setup is still handled by setup itself. Requires a lghorizon release with auth_failure_callback. Co-Authored-By: Claude Sonnet 5.5 <noreply@anthropic.com>
