English translation of ghq book

Please find an English translation of the ghq book in this pull request. It has been done with Google translate, and text validation, but I am confident it is a good translation (I worked for Toyota for 20 years, so did this sort of translation in work from Japanese to English).

Some key points:

• I would appreciate credit for the translation, but if you proceed to publish, any funds raised are yours. I don't want anything back from doing this work.
• I have left the "Published" boxes as "true" in the different pages of the document. I have no experience of publishing with leanpub, and I am not clear whether these have a function during the publishing steps.
