Evaluation tracing

• Add support for recording and outputting evaluation traces. Feature description and usage examples documented in https://github.com/cqframework/clinical_quality_language/wiki/Evaluation-tracing.
• Use the same TraceFrame class in both evaluation traces and backtraces.
• Fix context name and value types. The correct type is Pair<String, Any?>.
