Bump required CMake version to 3.9 to get Doxygen support out of the box

Add sub-folders EXAMPLE and FORTRAN. Write all warnings into "doxygen.log" to not clutter the output.

Initial support for Doxygen when building SuperLU with CMake.
I extracted all relevant fixes from #25. With CMake 3.9 calling Doxygen became much easier, so I wrote my own code. Not all Doxygen fixes are still required with Doxygen 1.9.
